Our Continuous Care Program is dedicated to transforming chronic care management by offering a suite of personalized services designed to improve patient outcomes and quality of life. Our mission is to alleviate the challenges faced by chronic care patients by delivering exceptional, personalized care and reducing the costs associated with medication and testing. We are committed to making a meaningful impact on the lives of our patients, ensuring they receive the highest standard of care and support.

ABOUT Zuri Health
Zuri Health is a virtual hospital (Super Health App) providing affordable and accessible healthcare services to patients across Sub-Saharan Africa via mobile app, website, Whatsapp bot, and SMS service. Through our platforms, patients can chat and consult with doctors, buy medication from pharmacies, book laboratory and diagnostic tests and even have a doctor visit them at home. With Zuri Health, the under-served populace can now access affordable and sustainable healthcare via SMS service to take care of the over 65% of the population without access to smartphones or the internet.
